1. 4 Passport size photos of each parent
2. 6 Passport size photos of the child
3. Aadhar Card (Parents & Child)
4. Caste Certificate (if applicable)
5. Health / Immunization Certificate
6. Proof of address (Lease Agreement/Utility Bills)
7. Report Card of last examination/school
8. Self-attested Date of Birth Certificate (Nursery to I)
9. Transfer Certificate (Class II onwards) + Attested DOB Copy

Duly filled and signed forms will be considered as incomplete for admission consideration in case of non submission of requisite mandatory documents.
